Preference Sheet

Vacations are to be taken in blocks of 14 or 21 days (note: if you have more than one week off rotation, you will be required to repeat that rotation again to obtain credit). Once you have given us your choice of vacations (as you will list below), you will not be permitted to change dates selected unless requested and approved three (3) months prior to the first day of the month of your vacation.  In addition, an email will be sent to you three (3) months in advance to confirm each vacation requested. A response to that email is mandatory. Failure to respond will result in your vacation request being denied.

The month you would like vacation must be selected now even if you do not have specific dates selected.  Vacation month may not be changed, only the dates within the month may be changed.  Vacations are to be taken in (1) two week block and (1) one week block or (1) three week block.  Please let us know your schedule for the academic year to ensure adequate coverage.

Vacation Rules:

• No vacation in December for Interns
• No vacation requests in June for PGY1 and PGY2
• PGY3's only allowed one week vacation in June
• Not all vacation requests will be approved.  We will try and honor at least 1 request.
• Interns must take two week block of vacation during Ambulatory medicine rotation.
